Reset Your LifeBe Proactive | Type 2 Diabetes Can Be Prevented or Delayed Reset Your Life is a Lifestyle Modification Program for the Prevention of Type 2 Diabetes supported by the Federal Government and Australian General Practice Network (AGPN). The program was developed by the Baker IDI Institute in 2008, and is currently being run at Adelaide Western General Practice Network (AWGPN). The program consists of seven sessions over a 6 month period, covering topics such as what diabetes and pre diabetes are, the implications it may have, diet and nutrition, exercise, goal setting, motivation, stress management, as well as smoking and alcohol cessation. Over the 6 month period participants are supported to learn about and implement individualised lifestyle changes to reduce their risk of developing type 2 diabetes. To be eligible participants must be between 40 to 49 years of age, (Indigenous 15 to 54), score 15 or above on the Australian Diabetes Risk Evaluation tool (AUSDRISK tool), they may be glucose intolerant but not currently diagnosed as having diabetes. (Participants who do not meet these criteria may be eligible to self fund for the program). Eligible participants can be referred to the program by their regular GP on the referral form provided. STOP Press - May 2010 The eligibility criteria for participants to the RESET YOUR LIFE program has been increased dramatically with the announcement that the AUSDRISK score has now been lowered to 12 from 15. This now enables people who are aged 40 to 49 or ATSI 15 to 54, who are at an intermediate risk of developing Type 2 Diabetes to be referred to the program through either the new 701, 703 or 715 health assessment.
