Australian Primary Care Collaborative Program

"If you always do what you have always done,

then you will always get what you have always got"


Congratulations to our 5 practices that have completed the 18 month Australian Primary Care Collaborative Program.

Participants included Kurralta Park Surgery, Garden Clinic, Croydon Medical Centre, Dr Moschou¿s, and Woodville South Medical Centre. AWGPN held a presentation evening on 30th of March 2010 to recognise the achievements of these practices. Click here to see photos

As part of the WA, SA and NT Wave of the collaborative program they have been involved in providing improved health outcomes for more than 10,000 Australians living with Diabetes and 6,500 Australians living with coronary heart disease, as well as making outstanding progress in the area of patient access and care redesign. This has lead to 800 more Australians living with diabetes having their cholesterol within recommended limits and, and approximately 900 more diabetics with their blood pressure within recommended limits.

Some improvements made by AWGPN practices include > 

> Increase in anti-platelet therapy for CHD patients from 60% to 80%

> Improved waiting times down from 3 days wait to 1 day for the GPs next third available appointment.

> Improved patient satisfaction with 30% improvement in patients getting an appointment with the practitioner of their choice on the day of choice.


The Australian Primary Care Collaborative Program (APCC) is a national program, which aims to provide practices with the skills to develop their management of patients care, and helps practices to tidy up data bases, implement improved patient access and encourage team building and communication between all practice staff.

Over an 18 month period practices have a GP and another staff member attend 3 pier led workshops, where they learn about how to utilize the improvement model and Plan Do Study Act (PDSA )cycles. Practices then submit PDSA and data to a secure online site once a month which enables them to measure their individual improvement as well as benchmark against other practices from the network, as well as nationally.

The program topics cover the areas of Access (goal to improve GP access for chronic disease patients), Diabetes and Chronic Heart Disease. In 2009 the APCC also introduced new measures of COPD and Prevention for those practices that have already completed the first level of the program.

As at Jan 2010 AWGPN has had 14 practices participate in this exciting program which has delivered significant Quality Improvement.

Also, Improvement Foundation Australia has introduced two new topics to the APCC Program.
The new topics, Chronic Obstructive Pulmonary Disease (COPD) and Chronic Disease Prevention and Self-Management (CDPSM) are being introduced to practices through a national APCC Program wave. Please see APCC 2 New Topics Flyer for more information.
