Denise has recently changed roles at the AWGPN, moving from Practice Nurse Coordinator to this newly created position.This is an exciting role which will not only integrate our programs internally, but also working on how best we can streamline / integrate our practice services; therefore ensuring we can respond more appropriately and efficiently to our GP members and practices. AWGPN Lifestyle Modification Coordinator, Felicity Steer
We welcome Felicity Steer who has been employed to manage the Lifestyle Modification Program and the APCC. 'Reset Your Life' is a new lifestyle management program for 40 to 49 year olds, which targets patients at risk of developing Type 2 Diabetes. The program encompasses areas such as, diet, nutrition, exercise smoking and motivation to make realistic lifestyle changes. Practices can refer patients who are identified by the Ausdrisk questionnaire as a high risk of developing Type 2 Diabetes, by either the new  Type 2 Diabetes Risk Evaluation (713), 45 - 49 year old health assessment (717), or ATSI (710) assessment MBS item numbers.
